2. Your Phone # in case of any questions
3. Your Mailing Address where you would like to have your receipt sent
4. Name you would like listed on the We Care Donor Wall* – See below for Examples or if you prefer to stay anonymous, please put “N/A”

i.e. In Honor of John & Jane Doe’s Grandchildren
*We Care Donor Wall is a list of donors names recognized on a designated page on the C.A.R.E website. Only names as designated by the donor are listed. No donation amounts are listed.

How do I get a receipt for my donation?
A receipt will be mailed to the address that you have provide as listed above.

Is this donation tax deductible?
Wyndmere C.A.R.E. has a 501 (c) 3 designation from the IRS. All donors’ gifts for the daycare project may be tax deductible depending upon your tax situation. Please consult your tax preparer.
